Supported Disability Accommodation Services

The Hume Region Aged & Disability Palliative Resource Nurses provides support to staff working in Supported Disability Accommodation Services or group homes to enable them to support and care for people with life-limiting illness and at the end of life.

Support could include but is not limited to providing information about palliative and end of life care, promoting links with community palliative care services and palliative care advice services, accessing suitable training and education resources, assistance with developing and implementing processes to support palliative and end of life care within the group home setting.
