  • Practical support for everyday activities;
  • Equipment loans;
  • Individual and family emotional support;
  • Bereavement care; and
  • After hours phone advice from palliative care nurses.

Our team work together with many other health care providers including GP’s, Medical Specialists and our local District Nursing Services in Wangaratta, Beechworth, Mount Beauty and Myrtleford to provide care and support to people with life limiting, chronic and oncological illness and their family members and carers.

We work with Caritas Christi Hospice (St Vincent’s Health) who provide afterhours phone advice for our clients, their family and carers.

Our team is supported with Secondary Specialist Palliative Care Consultancy by the East Hume Palliative Care Consultancy Service at Albury Wodonga Health, and St Vincent’s Health Melbourne.
